We help clients attend and follow up on appointments with their chosen physiotherapists, occupational therapists, speech pathologists, and more. Our team can accompany clients, provide reminders, and assist with notes and understanding next steps. |
We can support access to basic nursing services such as wound care, continence support, or chronic disease management through trusted registered nurses engaged under subcontracted or brokered arrangements. Our team helps coordinate visits and ensure the individual’s goals and preferences are respected. |
We support clients who self-manage their medications by providing gentle reminders and wellness check-ins as part of their daily routines. |
While not clinical, we assist with basic health tracking tasks like hydration reminders, food intake recording, and flagging wellbeing concerns to nominated health professionals. |
We guide clients to access the right registered clinicians for their care and provide administrative support in booking or communicating with providers. |
